This letter serves as a SECOND REQUEST under the Texas Public Information Act (Texas Gov’t Code §552.001 et seq.).
An initial request was submitted on December 15, 2025. However, this follow-up ensures statutory compliance and proper processing.
I hereby request all licensure, credentialing, employment-affiliation, and disciplinary records maintained by the Texas Board of Nursing for all nursing personnel who provided services at Williamson County Jail during the period:
January 1, 2024 through July 15, 2025
This request includes nurses employed by or assigned through multiple agencies, including Williamson County Sheriff’s Office, HealthSource, VitalCore, Adelphi Medical Staffing, and GreenLife Healthcare Staffing.
Scope of Nursing Records Requested
For each nurse, the following records are requested:
- Licensure records and status
- Employment and affiliation records
- Disciplinary and investigative records
- Public nurse profile documentation
Facility Coverage and Assignment Scope
