Scope of Texas Public Information Request EMS Records

RE: Public Information Request — EMS Provider Agency Records for All Entities Connected to Williamson County Jail (2024–2025)

Pursuant to the Texas Public Information Act (Texas Gov’t Code §552.001 et seq.), I request all EMS Provider Agency licensure, affiliation, and regulatory records for any agency that listed Williamson County Sheriff’s Office, Williamson County Jail, HealthSource, VitalCore, or Adelphi/GreenLife Staffing as:

  • an employer
  • a contracted service location
  • a practice location
  • a patient-care setting

between January 1, 2024 – July 15, 2025.

Records Requested Under Texas Public Information Request EMS Records

For every EMS Provider Agency with personnel assigned to or operating at the Williamson County Jail, produce:

1. EMS Provider Agency License Records

  • License number
  • Level (BLS, ALS, MICU, etc.)
  • Authorized service area
  • Issue/expiration dates
  • Compliance history
  • Any disciplinary actions

2. Personnel Rosters Filed with DSHS

  • Names of all EMTs, AEMTs, Paramedics, and LPs assigned
  • Active and inactive personnel lists
  • Credential levels
  • Agency affiliations

3. Complaints, Investigations, or Disciplinary Actions

Related to jail assignments, unlicensed practice, improper delegation, or medication administration.

4. Provider Applications, Renewals, and Amendments

Showing staffing of correctional or detention facilities.

Please include any EMS agency listing:

  • Williamson County Sheriff’s Office (WCSO)
  • HealthSource
  • VitalCore Health Strategies
  • Adelphi Medical Staffing
  • GreenLife Healthcare Staffing
  • Any EMS contractors hired by WCSO

This list is illustrative; therefore, include all connected agencies.
