RE: Second Public Information Act Request — Nursing Licensure, Credential, and Disciplinary Records for Personnel Assigned to Williamson County Jail (January 1, 2024 – July 15, 2025)

This letter serves as a SECOND REQUEST under the Texas Public Information Act (Texas Gov’t Code §552.001 et seq.).

An initial request was submitted on December 15, 2025.
This follow-up is submitted to ensure timely processing and statutory compliance.

I hereby request all licensure, credentialing, employment-affiliation, and disciplinary records maintained by the Texas Board of Nursing for all LVNs, RNs, APRNs, psychiatric nurses, nurse supervisors, and contract nursing personnel who provided medical or mental-health services at the Williamson County Jail during the period:

January 1, 2024 through July 15, 2025

This request includes nurses employed by or assigned through:

  • Williamson County Sheriff’s Office
  • HealthSource
  • VitalCore Health Strategies
  • Adelphi Medical Staffing
  • GreenLife Healthcare Staffing
  • Any subcontractor or temporary nursing agency

I. Records Requested

For each nurse assigned to or operating at the Williamson County Jail during the specified period, please produce:

1. Nursing Licensure Records

  • Full legal name
  • License number
  • License level (LVN, RN, APRN, PMHNP, etc.)
  • Issue and expiration dates
  • Compact status (if applicable)
  • Current license status (active, inactive, expired, suspended, revoked)

2. Employment and Affiliation Records

  • All employers or agencies listed with the Board
  • Any records reflecting assignment to correctional or detention facilities
  • Updates or amendments reflecting changes during the requested period

3. Disciplinary, Enforcement, or Investigative Records

  • Complaints filed
  • Investigations opened
  • Board actions
  • Final orders, sanctions, probations, suspensions, or revocations

4. Public Nurse Profile Documents

As maintained by the Texas Board of Nursing and subject to disclosure under Texas law.


II. Scope of Search (Not Exhaustive)

Please include all nurses assigned to any housing unit, medical clinic, transport detail, intake area, or mental-health service within the Williamson County Jail, including but not limited to:

  • R-24
  • P-24
  • B-5
  • B-7
  • B-9
  • I-13
  • C-14
  • All intake, clinic, transport, and vaccination activities

This list is illustrative and not exhaustive.

IV. Redaction Notice

Nursing license numbers, licensure status, and disciplinary actions constitute public information under Texas Gov’t Code §552.022(a)(17) and may not be redacted.


V. Withholding Notice

If any portion of the requested information is withheld, please identify each withheld record individually and cite the specific statutory exemption and responsible custodian in accordance with §552.301(e-1).


Please note that under Texas Government Code §§552.221 and 552.301, the Board is required to either produce the requested records or seek an Attorney General ruling no later than the 10th business day after receipt of this request.

Please confirm receipt of this second request and provide the estimated production timeline as required by law.
