This email serves as a SECOND REQUEST under the Texas Public Information Act (Texas Gov’t Code §552.001 et seq.).
An initial request was submitted on December 15, 2025. This follow-up request is submitted to ensure timely processing and statutory compliance.
I hereby request all EMS Provider Agency licensure, affiliation, and regulatory records for any EMS provider agency that listed any of the following as an employer, contracted service location, practice location, or patient-care setting during the period January 1, 2024 through July 15, 2025:
