Good afternoon,
This email serves as a SECOND REQUEST under the Texas Public Information Act (Texas Gov’t Code §552.001 et seq.).
An initial request was submitted on December 15, 2025.
This follow-up is submitted to ensure timely processing and statutory compliance.
I hereby request all EMS Provider Agency licensure, affiliation, and regulatory records for any EMS provider agency that listed any of the following as:
- an employer,
- a contracted service location,
- a practice location, or
- a patient-care setting
during the period January 1, 2024 through July 15, 2025:
- Williamson County Sheriff’s Office
- Williamson County Jail
- HealthSource
- VitalCore Health Strategies
- Adelphi Medical Staffing
- GreenLife Healthcare Staffing
I. Records Requested
For each EMS Provider Agency with personnel assigned to or operating at the Williamson County Jail, please produce:
1. EMS Provider Agency License Records
- License number
- Service level (BLS, ALS, MICU, etc.)
- Authorized service area
- Issue and expiration dates
- Compliance history
- Any disciplinary actions
2. Personnel Rosters Filed with DSHS
- Names of all EMTs, AEMTs, Paramedics, and Licensed Paramedics
- Active and inactive personnel lists
- Credential levels
- Agency affiliations
3. Complaints, Investigations, or Disciplinary Actions
Related to jail assignments, unlicensed practice, improper delegation, or medication administration.
4. Provider Applications, Renewals, and Amendments
Reflecting staffing of correctional or detention facilities.

Please include all EMS provider agencies that listed or affiliated with:
- Williamson County Sheriff’s Office (WCSO)
- HealthSource
- VitalCore Health Strategies
- Adelphi Medical Staffing
- GreenLife Healthcare Staffing
- Any EMS contractor engaged by WCSO

V. Withholding Notice
If any portion of the requested information is withheld, please identify each withheld record individually and cite the specific statutory exemption and responsible custodian in accordance with §552.301(e-1).
Please note that under Texas Government Code §§552.221 and 552.301, the Department is required to either produce the requested records or seek an Attorney General ruling no later than the 10th business day after receipt of this request.
