RE: Passport Records Request & Passport Alert Flag – Minor Child Verification
Minor Child (Initials Only): ILN
DOB: May 30, 2017
Parent/Requester: LeRoy Nellis
To Whom It May Concern:
I am formally requesting, under the Privacy Act of 1974 and the Freedom of Information Act, verification regarding whether a U.S. passport has ever been issued, renewed, applied for, or is currently pending for my minor child, herein identified as ILN.
I do not possess any of ILN’s identifying documents, as these have been withheld by the child’s mother.
DIVORCE DECREE PASSPORT RESTRICTION
My divorce decree explicitly states no U.S. passport may be issued for ILN without my written consent.
FOREIGN CITIZENSHIP & FLIGHT-RISK DISCLOSURE
The child’s mother, Isolde Amador-Melendez, is a citizen of Nicaragua and a lawful permanent resident of the United States, presenting elevated risk of international removal or concealment.
REQUEST FOR IMMEDIATE PASSPORT ALERT FLAG
That ILN’s passport file be immediately flagged for parental-abduction risk and no passport be issued or renewed without my written consent.
INFORMATION REQUESTED
- Confirmation whether a passport has ever been issued for ILN.
- Confirmation of any pending or active application.
- Copies of applications or parental-consent forms.
- Immediate alerts regarding unauthorized passport activity.
Respectfully,
/s/ LeRoy Nellis
