Assigned to Williamson County Jail, Jan 2024 – Jul 2025
Pursuant to the Texas Public Information Act (Texas Gov’t Code §552.001 et seq.), I request complete licensure, certification, and disciplinary records for all EMTs, AEMTs, Paramedics, Licensed Paramedics, EMS Medical Technicians, nurses, contract medics, and any DSHS-regulated medical personnel who provided services at the Williamson County Jail between:
January 1, 2024 – July 15, 2025
This request covers all personnel employed by or contracted through:
• Williamson County Sheriff’s Office (WCSO)
• HealthSource
• VitalCore
• Adelphi Medical Staffing
• GreenLife Healthcare Staffing
• Any third-party or temporary medical-staffing entity
I. Records Requested
For every DSHS-licensed medical or EMS individual assigned to the Williamson County Jail during the above period, please produce:
1. Full licensure record
● Full legal name
● Certification type & level (EMT, AEMT, Paramedic, LP)
● License / Certification number
● Issue date & expiration date
● Current status (active, inactive, expired, suspended, revoked)
2. Complete public disciplinary history
● Notices of investigation
● Complaints filed
● Final actions or sanctions
● Restrictions, probated actions, or revocations
3. Affiliated EMS Providers & Employment Listings
● All agencies/employers listed with DSHS for each individual
● Any records indicating assignment to Williamson County Sheriff’s Office or HealthSource/VitalCore
4. DSHS Personnel File Documents (Public Portions)
● Certification printouts
● Public EMS personnel record sheets
● National Registry (NREMT) public credential confirmations (if available)
5. Known Individuals (Not Exhaustive)
Please include, at minimum, the following:
● EMT David A. Miller
● EMT Paige P. Altobelli
● EMT/Medic Larry R. Davis
● EMT/Medic Joshua D. Ransom
● Any EMTs assigned to units R-24, P-24, B-5, B-9, B-7, I-13, C-14
● Any EMT/Paramedic listed on COVID-19 vaccine logs
This list is illustrative only — please include all medical/EMS personnel on record.

III. Redaction Notice
Under Texas Gov’t Code §552.022(a)(17), professional license numbers and all disciplinary actions are public information.
These may not be withheld or redacted.
IV. If Any Records Are Withheld
Identify each withheld document individually and cite the specific statutory exemption under §552.301(e-1).
Provide written certification stating:
● why it was withheld,
● who is the custodian,
● and what statutory provision is invoked.
V. Public Interest Statement
This request directly concerns:
• credential compliance,
• public-safety oversight,
• scope-of-practice enforcement, and
• state-licensed medical activity inside a Texas jail.
These matters are of clear and overriding public interest and are not subject to privacy exemptions.
