Scope of Texas BON Nurse Records Request

RE: Public Information Request — Nurse Licensure & Disciplinary Records for All Nurses Assigned to Williamson County Jail (2024–2025)

Under the Texas Public Information Act, I request complete licensure and disciplinary records for all LVNs, RNs, APRNs, psychiatric nurses, and nursing supervisors assigned to the Williamson County Jail Medical Unit between January 1, 2024 – July 15, 2025.

Entities and Staffing Coverage

This request includes personnel employed by or contracted through the following entities:

  • Williamson County Sheriff’s Office
  • HealthSource
  • VitalCore
  • Adelphi Medical Staffing
  • GreenLife Healthcare
  • Any subcontracted nursing entity

Records Requested Under Texas BON Nurse Records Request

For every nurse who provided care during this period, please produce:

Licensure Verification Records

  • Full legal name
  • License number
  • Level (LVN, RN, APRN, PMHNP)
  • Issue, renewal, and expiration dates
  • Compact status
  • Current license status

Disciplinary and Enforcement Records

  • Complaints
  • Investigations
  • Final orders
  • Board actions
  • Any probation, suspension, remediation, or revocation

Employment and Affiliation Records

  • All employers listed with BON
  • Employer updates showing WCSO, HealthSource, or VitalCore

As defined under Occupations Code §301.158 and §552.022(a)(17).
