Scope of DSHS Licensure Records Request

RE: Public Information Request — DSHS Licensure Records for All Medical Personnel Assigned to Williamson County Jail, Jan 2024 – Jul 2025

Pursuant to the Texas Public Information Act (Texas Gov’t Code §552.001 et seq.), I request complete licensure, certification, and disciplinary records for all EMTs, AEMTs, Paramedics, Licensed Paramedics, EMS Medical Technicians, nurses, contract medics, and any DSHS-regulated medical personnel who provided services at the Williamson County Jail between January 1, 2024 – July 15, 2025.

Entities and Contractors Included

This request covers all personnel employed by or contracted through:

  • Williamson County Sheriff’s Office (WCSO)
  • HealthSource
  • VitalCore
  • Adelphi Medical Staffing
  • GreenLife Healthcare Staffing
  • Any third-party or temporary medical staffing entity

Records Requested Under DSHS Licensure Records Request

For every DSHS-licensed medical or EMS individual assigned to the Williamson County Jail, please produce:

Full Licensure Records

  • Full legal name
  • Certification type and level (EMT, AEMT, Paramedic, LP)
  • License or certification number
  • Issue and expiration dates
  • Current status (active, inactive, suspended, revoked)

Disciplinary and Investigation Records

  • Notices of investigation
  • Complaints filed
  • Final disciplinary actions or sanctions
  • Restrictions or revocations

Agency Affiliations and Employment Records

  • All agencies or employers listed with DSHS
  • Assignments tied to Williamson County Jail or contractors
