Dear Officials,

My name is LeRoy Nellis II, and I am submitting a public-health and medical-licensing complaint regarding an incident that occurred in March–April 2024 in Williamson and Travis Counties, Texas.

During that period I was injected with approximately 50 cc of a COVID-19 vaccine by an EMT identified to me as Larry R. Davis (Texas EMS License #1566355). The injection was not performed in a clinical setting and far exceeded approved dosage limits. I experienced adverse physical effects following this event.

Because this occurred during a local disease outbreak reported in both counties, I am requesting a review of whether any public-health or emergency-response irregularities took place.

Additional personnel whose state licenses were located through the Texas Department of Public Safety / Texas Commission on Law Enforcement are listed below for reference only, to assist investigators in verifying records or potential involvement:

  • Derek W. Garretson – Texas License #1380862
  • Nathan J. Henderson – Texas License #1404126

I also documented statements by two law-enforcement officers referencing the event (“reading all about it … can’t believe he didn’t get sick”). I am asking for:

  1. A CDC VAERS safety investigation into the vaccine administration incident.
  2. An FDA Office of Regulatory Affairs inquiry into product misuse.
